SPRING 2018 REGISTRATION
Use this form to register for EAS Spring 2018 classes, read and sign our school indemnity waiver, and complete the medical release. Full class schedules can be found at our website here. GENERAL REGISTRATION INFO
1) There must be a minimum of 7 students enrolled for a class to continue (with the exception of private & semi-private lessons). There is maximum of 15 students allowed in each class (unless approved by instructor). If the class does not meet the minimum student requirement the class may be cancelled. Notification of class cancellations will be sent to students only if they have pre-registered. If a student pre-registers for a class that has been cancelled due to low enrollment, they will have a choice of choosing another class or receiving a full refund for the cancelled class.
2) We have a dress code for our dance classes and all aerial classes. Full description and cost (if applicable) can be found here.
3) Shows: At the conclusion of the semester all students will participate in a stage production, film, and/or gallery, allowing them to demonstrate what they have learned during the semester. Rehearsal dates and times will be determined closer to the performance, and will be posted on Facebook and our website. (does not apply to non-performing classes)
Paid in full at the time of registration. (to include registration fee, production fee, and costuming fee - if applicable)
Monthly payment option will add an additional $20 per family per semester. This will apply to our payment plan of 4 payments per semester. Registration fee (if applicable), production fee and costuming fee will be due at the time of registration along with 25% of the tuition amount. Payment plans are required to keep a credit card on file or post-dated checks.
Payments are due by the 5th of each month. If payment is not received by the 15th of the month, a late fee of $15 will be added to your payment. Accounts more than 30 days past due will result in your student being removed from classes until account is brought current. Please keep in mind as this may impact their ability to perform in the semester-end performance. Student will still be responsible for the full semester's tuition.
Returned check fee: Returned checks will result in a $25 fee added to your payment and the payment will need to be paid by credit card, cashier's check or cash.
$25 - this registration fee covers student administration and provides an EAS T-shirt.
This is an annual fee. (Always charged in the fall, unless new student)

COSTUME AND PRODUCTION FEE
Costume Fees will be charged for EACH performing class that a student is registered. $75 (child sizes) OR $85 (adult sizes). These costumes will be yours to keep!
Each student will be charged a $25 production fee. This fee will include a commemorative show t-shirt. It also allows the student to invite as many guests to the show as they would like.

First, thanks for partnering with us at EAS! We are expecting an amazing semester filled with fun, hard work, and growth. Second, we will be reviewing your registration and placing your student on the appropriate class rosters. Some classes fill up fast, especially aerials, so we will contact you if we need to make other arrangements due to class size. Finally, if you've been with us before you probably have an idea of how much your tuition will be; but if this is your first time, those calculations may be a bit scary. Don't worry! We'll contact you with tuition totals and help you either pay your balance in full or set up your payment plan.
Initial payments are due the first week of classes.
You will receive an emailed invoice with your total amount due. You can now pay online right from your invoice using your phone, computer or tablet. If you have any billing questions, please contact us at email@example.com
