SPRING 2018 REGISTRATION

 

Use this form to register for EAS Spring 2018 classes, read and sign our school indemnity waiver, and complete the medical release. Full class schedules can be found at our website here. GENERAL REGISTRATION INFO

1) There must be a minimum of 7 students enrolled for a class to continue (with the exception of private & semi-private lessons). There is maximum of 15 students allowed in each class (unless approved by instructor). If the class does not meet the minimum student requirement the class may be cancelled. Notification of class cancellations will be sent to students only if they have pre-registered. If a student pre-registers for a class that has been cancelled due to low enrollment, they will have a choice of choosing another class or receiving a full refund for the cancelled class.

2) We have a dress code for our dance classes and all aerial classes. Full description and cost (if applicable) can be found here

3) Shows: At the conclusion of the semester all students will participate in a stage production, film, and/or gallery, allowing them to demonstrate what they have learned during the semester. Rehearsal dates and times will be determined closer to the performance, and will be posted on Facebook and our website. (does not apply to non-performing classes)

PAYMENTS

Option #1

Paid in full at the time of registration. (to include registration fee, production fee, and costuming fee - if applicable)

Option #2

Monthly payment option will add an additional $20 per family per semester.  This will apply to our payment plan of 4 payments per semester.  Registration fee (if applicable), production fee and costuming fee will be due at the time of registration along with 25% of the tuition amount.  Payment plans are required to keep a credit card on file or post-dated checks.

Payments are due by the 5th of each month.  If payment is not received by the 15th of the month, a late fee of $15 will be added to your payment.  Accounts more than 30 days past due will result in your student being removed from classes until account is brought current.  Please keep in mind as this may impact their ability to perform in the semester-end performance.  Student will still be responsible for the full semester's tuition.

Returned check fee: Returned checks will result in a $25 fee added to your payment and the payment will need to be paid by credit card, cashier's check or cash.

 

REGISTRATION FEE

 $25 - this registration fee covers student administration and provides an EAS T-shirt.

This is an annual fee. (Always charged in the fall, unless new student)

COSTUME AND PRODUCTION FEE

Costume Fees will be charged for EACH performing class that a student is registered.  $75 (child sizes) OR $85 (adult sizes).  These costumes will be yours to keep!

Each student will be charged a $25 production fee.  This fee will include a commemorative show t-shirt.  It also allows the student to invite as many guests to the show as they would like.

Classes dropped prior to the start date will receive a full refund, less registration fee. Classes dropped prior to the end of the 4th week will receive a 50% refund, less registration fee. THERE IS NO REFUND FOR CLASSES DROPPED AFTER THE FOURTH WEEK. I have read and understand the above stated policy. All private lesson cancellations need to be made with the instructor 24 hours prior to the scheduled lesson. YOU WILL BE FINANCIALLY RESPONSIBLE FOR LESSONS THAT ARE CANCELLED WITH LESS THAN 24 HOURS NOTICE. I have read and understand the above stated policy. Subject(s), hereby grants to Experience Arts School Ministries the exclusive rights to use the video/photo(s) taken/supplied with Subject's image(s) for the inclusion in any work produced by Experience Arts School Ministries. Subject acknowledges that the depiction of him/her in the video/photo(s) may be duplicated, altered, and distributed in any and all manner and media throughout the world in perpetuity. Subject(s) hereby releases and discharges Experience Arts School Ministries, its employees, agents, licensees, successors, and assigns from any and all claim, demands or causes of actions that it may have from now on and for libel, defamation, invasion of privacy or the right of publicity, infringement of copyright or trademark, of violation of any other right arising out of or relating to a utilization of the rights granted under this agreement. Subject(s) agrees that Experience Arts School Ministries shall have the unlimited right to vary, change, alter, modify, add to and/or delete from his/her depiction in the video/photo(s) and to rearrange and/or transpose his/her depiction, and to use a portion or portions of his/her depiction of character together with any other literary, dramatic, or other material of any kind. All rights, licenses and privileges herein granted to Experience Arts School Ministries are irrevocable and not subject to rescission, restraint or injunction under any circumstances. Nothing herein shall be construed to obligate Experience Arts School Ministries to produce, distribute or use any of the rights granted herein. This agreement shall be construed according to the laws of the State of Arizona applicable to agreements which are executed and fully performed within said State. This agreement contains the entire understanding of the parties relating to the subject matter, and this agreement cannot be changed except by written agreement executed by the party to be bound. I have read and understand the above stated VIDEO AND PHOTO RELEASE. I understand that an electronic signature has the same effect and can be enforced in the same way as a written signature.
I have read, understood and will comply with all guidelines, procedures, and policies put forth by Experience Arts School. In consideration of your accepting me, or my child, for participation in Experience Arts School, I hereby for myself, my heirs, executors, and administrators, waive and release any and all rights and claims for damages that I may have against the aforementioned organization and its agents, employees, representatives, successors and assigns for any and all injuries suffered by myself or my child that arise out of the aforementioned program, activity, sport, dance sponsored by the aforementioned organization. I warrant that I have the right to authorize the foregoing and do hereby agree to hold the aforementioned organization harmless of and from any and all liability of whatever nature which may arise out of or result from such participation. For the consideration stated above, I further agree that in the event that my child or I should make any claim against the aforementioned organization for damages arising out of the aforementioned program, activity, sport or dance, I will personally indemnify, defend, and hold harmless the organization and its agents, employees, and representatives, successors, and assigns against any and all loss and damage occasioned thereby, including attorney's fees. I have read and understand this Agreement and have willingly placed my signature below as evidence of my acceptance of all conditions contained herein. YOU’RE ALMOST FINISHED

First, thanks for partnering with us at EAS! We are expecting an amazing semester filled with fun, hard work, and growth. Second, we will be reviewing your registration and placing your student on the appropriate class rosters. Some classes fill up fast, especially aerials, so we will contact you if we need to make other arrangements due to class size. Finally, if you've been with us before you probably have an idea of how much your tuition will be; but if this is your first time, those calculations may be a bit scary.  Don't worry! We'll contact you with tuition totals and help you either pay your balance in full or set up your payment plan. 

Initial payments are due the first week of classes.
